For #311, item 5 of #168. Cache hits, misses, upstream fetches, the bytes those fetches read and transforms are now Prometheus counters in memory: pixa_cache_hits_total, pixa_cache_misses_total, pixa_upstream_fetches_total, pixa_upstream_fetch_bytes_total and pixa_transforms_total. They are fields of the cache. NewCache makes them and registers them on the registry it is given, in internal/imgcache/counters.go. The Increment methods add to their cache's counters, so a cache hit still makes no database call.
pixad's fx graph provides Prometheus' default registry, the one /metrics serves, and the handlers pass it to NewCache. A registration error fails startup. Each test gives its cache a new registry.
Gone, with their tests: cache_stats, Cache.Stats, Service.Stats, CacheStats, Stats on ImageCache, and the pending counts and their write. The background goroutine writes access times as before; its count-named identifiers and its file are renamed to say so. README.md names the counters and no longer mentions writing counts.
Judgement call: pendingCountsCancel is renamed too, as its name would be false.
Judgement call: the two stop tests whose write under way was a count now use access times. TestStopPendingCountWrites_WritesPendingCounts is now TestStopPendingAccessTimeWrites_LetsTheWriteUnderWayFinish, as the plain rename is taken. The database package's concurrent-writes test drops its hit-count write.
The counters count whether or not metrics credentials are set, as the table did; only /metrics depends on them.
Unverified: make bench was not run, as this host lacks the libvips development files; the benchmark compiles in the test phase.

internal/imgcache/counters.go, and the seven tests marked //nolint:paralleltest. The counters are package-level variables. That needs one gochecknoglobals suppression and seven paralleltest suppressions, which a plain design avoids. The Go style guide says to avoid global state and use dependency injection, and AGENTS.md says each lint finding is to be fixed in the code. The repo already does this the plain way: internal/middleware takes the registry it registers with (metricsRegistry), and its tests pass a registry of their own. The suppression's reason, "Prometheus registers each counter once per process", is false: one registry refuses a second counter of the same name, but a new registry accepts it. Because the counters are global, IncrementStats and the other Increment methods ignore their cache and add to counters that every cache in the process shares. The tests then have to subtract a before reading and rely on the order Go runs non-parallel tests in. Acceptable: the five counters are fields of the cache, made in its constructor and registered on a registerer it is given. Production wires in Prometheus' default registry once; each test passes a new registry. No suppression, and the counting tests keep t.Parallel and read their own cache's counters.
internal/imgcache/pendingaccesstimes_internal_test.go.TestStopPendingCountWrites_WritesPendingCounts was deleted, not moved to access times. It was the only test of behaviour that stays: while the background writer runs, the stop waits for the write under way, then writes what was handed over after it. That is the shutdown path pixa takes in production, and now no test covers it. The PR body says two stop tests were moved to access times, but only TestStopPendingAccessTimeWrites_ReturnsWhenItsContextEnds was. The approval in #311 removes only the tests of removed code. Acceptable: keep the test, with access times in place of the counts, handed over before and after the write under way. It asserts that at most one write waited for the database and that both access times were written.
README.md, shutdown paragraph. "pixa then writes to the database the times cached images were last used that are not written yet (see Storage), and exits." The clauses are stacked: "that are not written yet" lands after "were last used", far from "the times". Acceptable: a short sentence such as "pixa then writes the waiting last-used times to the database (see Storage) and exits."
README.md, Storage paragraph.
"The time the image was last served, which eviction orders by, is handed to a background writer without waiting for it." The verb comes late, after the inserted clause, and the sentence never says what does not wait.
"A miss also hands that writer the time it last used a cached source image." Here "it" could be the miss or the writer, and a single miss has no "last" use.
Acceptable: short sentences that name the request, such as "A hit hands the time it was served to a background writer and does not wait for it. Eviction orders by that time. A miss that uses a cached source image hands that writer the time as well."
README.md, Metrics line. The main point, what /metrics holds, now comes after the edge cases of the labels (unmatched, other). Acceptable: the list right after the sentence saying when /metrics is served, and the label rules after the list.
Judgement call: the database package's concurrent-writes test dropping its write to cache_stats counts as following removed code. It is not a finding.

The five counts the cache_stats table held are now Prometheus counters in
memory, as fields of the cache. NewCache makes them and registers them on
the registry it is given. pixad gives it Prometheus' default registry,
which /metrics serves; each test gives it a new one. The Increment methods
add to their cache's counters, so counting makes no database call.
cache_stats, Cache.Stats, Service.Stats, CacheStats and the pending counts
are gone, with their tests. The background goroutine now writes only access
times, and its names say so: StartPendingAccessTimeWrites,
StopPendingAccessTimeWrites and pendingaccesstimes.go. The two stop tests
whose write under way was a count now use access times.
Model: opus-5-5
The counters are fields of the cache, made in NewCache and registered on the registry it is given. pixad provides the default registry through the fx graph; each test passes a new one. A registration error fails startup. Both suppressions are gone, and the counting tests run in parallel on their own cache's counters, with their original assertions.
Restored as TestStopPendingAccessTimeWrites_LetsTheWriteUnderWayFinish, since the plain rename is the name of an existing test.
Rewritten as suggested, naming the cached images whose times they are.
Rewritten as suggested; the miss sentence says it hands over the time it used the source image.
The list now follows the sentence on when /metrics is served; the label rules follow the list, in the same bullet.

PASS: this change fixes the five findings of the first review and meets the definition of done of #311.
Judgement call: the middleware still reaches Prometheus' default registry by leaving its registry field empty, while the handlers now take the registry from the fx graph. This is not a finding, as changing the middleware is outside this issue.
