A variable named in the config file's env: section is set while the
file loads, so it overrides both the environment the process was started
with and the file's own key. README.md and the config.example.yml header
said only that a variable wins over the file; they now state this
exception where the precedence is given.

Each config key can now be set by PIXA_ plus the key in upper case
("." written as "_"), and the port by PORT. One list pairs keys with
variables; the typed getters read a present variable, even an empty
one, before the config file, so every existing check covers it, and
errors a variable can reach name both the key and the variable. An
empty string for blocked_networks or trusted_proxies is now an empty
list, as for allowlist_hosts. The image no longer bakes in
config.docker.yml or passes --config, and its HEALTHCHECK probes
${PORT:-8080}; the config file is looked for under /etc/pixa rather
than /etc/pixad, so a file mounted at /etc/pixa/config.yml is still
read.

Tests, written before the change, for the PIXA_ variables and PORT:
every key set from the environment with no config file, PORT over the
file's port, a list variable replacing the file's list, an empty
variable as a set value, invalid values aborting startup naming the
variable, and the signing key and metrics password never printed. All
fail until the change lands, except the check that a config file alone
behaves as before. TestMain unsets PORT and every PIXA_ variable so the
shell running the tests cannot change their result.

Define ContentHash, VariantKey, and PathHash types to replace
raw strings, providing compile-time type safety for storage
operations. Update storage layer to use typed parameters,
refactor cache to use variant storage keyed by VariantKey,
and implement source content reuse on cache misses.
Add trailing filename to encrypted URLs for better browser compatibility.
The filename is ignored by the server but helps browsers identify content type.
Since signing_key is now required at config load time, sessMgr, encGen,
and signer are always initialized. Remove unnecessary nil checks that
were runtime failure paths that can no longer be reached.

Hot cache entries now store all data needed to serve a cache hit
without any database access:
- OutputHash (for file lookup)
- ContentType (for Content-Type header)
- SizeBytes (for Content-Length header)
Previously hot cache only stored OutputHash, causing empty
Content-Type headers on cached WebP responses.
When only width or height is specified (the other being 0), scale the
image proportionally to maintain aspect ratio. Previously, 0 was passed
directly to the resize function which produced a 0x0 image.
Uses github.com/gen2brain/webp - a CGO-free library that uses WASM via
wazero runtime for encoding. WebP decoding was already supported.

TDD: This test expects WebP encoding to succeed. It currently fails

Never silently fall back to a different setting when a user's parameter
explicitly specifies a value. Return an error for invalid explicit values;
only apply defaults for omitted parameters.
- Return ErrUnsupportedOutputFormat for WebP/AVIF encoding

Silent fallbacks violate the principle of least surprise and mask bugs.
When a user explicitly specifies a value, we should either honor it or
return an error - never silently substitute a different value.

Implements the Processor interface using disintegration/imaging library.
Supports JPEG, PNG, GIF, WebP decoding and JPEG, PNG, GIF encoding.

Migrations are stored in schema/*.sql and embedded via go:embed.
Applied migrations are tracked in schema_migrations table.
Initial schema includes source_content, source_metadata, output_content,
request_cache, negative_cache, and cache_stats tables.
