The new package internal/providers builds the cache, the fetcher, the
processor, the image service, the session manager and the encrypted URL
generator from the config. pixad's fx graph provides them with
providers.Constructors(), and handlers.New receives them built. The
server tests build the same graph, with only the fetcher's dial function
replaced. database.New opens the database, so these parts can read it
while fx builds them; the cache's stop hook still runs before the
database's.
Each setting's default is declared in internal/config, and
internal/providers works out the default cache_max_bytes. The cache's
MaxBytes is the one value for the disk cache limit, and 0 turns the disk
cache off. The fetcher and the processor take explicit values.

internal/database now opens the database with one connection. pixa's
own reads and writes run on it one at a time instead of competing for
SQLite's lock, where a write that kept losing could wait past the
five-second busy timeout and fail with "database is locked". The busy
timeout stays, for another program writing to the same file.
With one connection, a query run while rows or a transaction are still
open would wait forever. No code in internal/database or
internal/imgcache does that; the comment on DB() tells callers.
README.md says pixa uses one connection and that requests wait while
eviction runs one of its queries.
Model: opus-5-5
Requests and the eviction pass write on separate connections, and with
no busy timeout a write that met another one failed at once with
"database is locked" and was lost. internal/database now adds
_pragma=busy_timeout(5000) to every db_url, the default or one the
operator sets, so such a write waits up to five seconds. The default
db_url's _journal_mode=WAL is not a parameter the driver reads, so it
is now _pragma=journal_mode(WAL). README.md and config.example.yml say
what pixa adds to db_url.
Model: opus-5-5
REPO_POLICIES.md puts migrations in internal/db/migrations/ as
000_migration.sql and 001_schema.sql. The two files move there with
their contents unchanged. go:embed cannot reach outside its own
package, so internal/db/migrations has a small package that embeds
them, and internal/database reads them through its FS(). The database
package stays where CONVENTIONS.md puts it; moving it would change
existing test files in other packages.
The version still comes from the filename prefix, so a database that
has recorded versions 0 and 1 runs neither again. A new test applies
the migrations twice to one database file and checks that the second
run applies nothing.

Accumulating milestone branch. One squashed commit per closed issue; `next` is kept green and mergeable to `main` at any time without notice.
Landed so far:
- `chore: update golangci-lint to v2.12.2 with canonical config` (#54) — canonical v2-schema `.golangci.yml`, pins bumped in `Dockerfile` and `script/bootstrap`, tree at `0 issues.`. Three behaviour deltas are recorded in that PR's body: `Cache.StoreVariant` takes a context, `MetadataStorage.Store` no longer leaks temp files on failure, and the `signing_key` too-short error text gained a `value too short:` prefix.
Sequencing for the milestone is tracked in #103.

## Summary
Moves the `schema_migrations` table definition from inline Go code into `internal/database/schema/000.sql`, so the migration tracking table schema lives alongside all other schema files.
closes#29
## Changes
### New file: `internal/database/schema/000.sql`
- Contains the `CREATE TABLE IF NOT EXISTS schema_migrations` DDL
- This is applied as a bootstrap step before the normal migration loop
### Refactored: `internal/database/database.go`
- Removed the inline `CREATE TABLE IF NOT EXISTS schema_migrations` SQL from both `runMigrations` and `ApplyMigrations`
- Added `bootstrapMigrationsTable()` which:
- Checks `sqlite_master` to see if the table already exists
- If missing: reads and executes `000.sql` to create it, then records version `000`
- If present (backwards compat with existing DBs created by old inline code): back-fills version `000` so the normal loop skips the bootstrap file
- Deduplicated: both `Database.runMigrations()` and the exported `ApplyMigrations()` now delegate to a single `applyMigrations()` helper
- Added `logInfo`/`logDebug` helpers to handle the optional logger (nil when called from `ApplyMigrations` in tests)
### New file: `internal/database/database_test.go`
- `TestApplyMigrations_CreatesSchemaAndTables` — verifies all migrations apply and all expected tables exist
- `TestApplyMigrations_Idempotent` — verifies running migrations twice produces no errors or duplicates
- `TestBootstrapMigrationsTable_FreshDatabase` — verifies bootstrap creates the table and records version 000
- `TestBootstrapMigrationsTable_ExistingTableBackwardsCompat` — verifies existing DBs (from old inline-SQL code) get version 000 back-filled without data loss
## Conflict note
[PR #33](#33) (for [issue #28](#28)) is also modifying migration code. This PR is based on current `main` and the conflict will be resolved at merge time.

Closes#28
Migration filenames now follow the pattern `<version>_<description>.sql` (e.g. `001_initial_schema.sql`). The version stored in `schema_migrations` is the numeric prefix only, not the full filename stem.
## Changes
- **`ParseMigrationVersion()`** — new exported function that extracts the numeric prefix from migration filenames. Validates that the prefix is purely numeric and rejects malformed filenames (empty prefix, non-numeric characters, leading underscore).
- **Renamed `001.sql` → `001_initial_schema.sql`** — migration files can now have descriptive names while the tracked version remains `001`. This is safe pre-1.0.0 (no installed base).
- **Deduplicated migration logic** — `runMigrations()` and `ApplyMigrations()` now share a single `applyMigrations()` implementation, plus extracted `collectMigrations()` and `ensureMigrationsTable()` helpers.
- **Unit tests** — `TestParseMigrationVersion` covers valid patterns (version-only, with description, multi-digit, multiple underscores) and error cases (empty, leading underscore, non-numeric, mixed alphanumeric). `TestApplyMigrations` and `TestApplyMigrationsIdempotent` verify end-to-end migration application against an in-memory SQLite database.

Migrations are stored in schema/*.sql and embedded via go:embed.
Applied migrations are tracked in schema_migrations table.
Initial schema includes source_content, source_metadata, output_content,
request_cache, negative_cache, and cache_stats tables.
