SPEC.md is rewritten: every requirement once, with its reason; the two deferred points under "Planned, not built"; the reference material in docs/ (settings, log fields, metrics, image, formats), shared byte-identical with #157. Where the old spec disagreed with the code, the new one follows it:
off: the old spec allowed it for every limit and threshold but one. The code takes it only for the settings listed under "Settings"; timeouts for lookups and reputation, ban durations, state intervals, counts and percentages refuse it.
lookups.json: deferred in the old spec; written and read by the code.
Rate limit passed: 429 in the old build order; a ban in the code.
Clients endpoint: reputation in the old spec; counters, history and bans with notes in the code.
Trap paths: unplaced in the old data flow; after rate limiting, before rule files.
header: target: Host and Transfer-Encoding refused.
Error burst: trap paths count too.
Memory bounds: reputation verdicts and anomaly counters too.
SWWAF_BLOCKLIST_REFRESH: less than 1h refused.
Alert cooldown: keyed by netblock and source, file or thing counted.
Added: SWWAF_IPV6_GROUP_PREFIX's range; off for the alert cooldown; trap path in ban notes; refusals and dropped alerts in the metrics; the summary event; golang-lru and mmdbwriter listed; the package list.
Dropped as history or operator advice (README.md has it): milestones and build order, "Rollout per service", the example Dockerfile. "Risks the design has to handle" is gone; its two points stated nowhere else moved to "Attack detection" and "Anomaly thresholds".

`SPEC.md` is rewritten: every requirement once, with its reason; the two deferred points under "Planned, not built"; the reference material in `docs/` (settings, log fields, metrics, image, formats), shared byte-identical with https://git.eeqj.de/sneak/smallwebwaf/pulls/157. Where the old spec disagreed with the code, the new one follows it:
- `off`: the old spec allowed it for every limit and threshold but one. The code takes it only for the settings listed under "Settings"; timeouts for lookups and reputation, ban durations, state intervals, counts and percentages refuse it.

Model: fable-5-1
The old SPEC opened with a draft status and milestone narrative, said
several requirements twice, and ended with a build order that is all
built. The new one keeps every requirement the old one states, said
once, in short sentences with bulleted lists in place of comma chains.
What the code on next does is in the main sections; the two points the
old SPEC deferred, sharing bans between instances and RELP, are in a
last section, "Planned, not built", each marked as planned. The status
line, the milestones and the build order are dropped as history. Where
the old SPEC disagreed with the code, the new one follows the code; the
PR body lists each such point.

1. Not short, and many requirements are said two to four times. The new spec has about 15,100 words; the old one had about 16,500. #156 asks for a short spec that says each requirement once. Requirements said more than once:
the 2026 defaults (Purpose, Settings);
the SWWAF_ prefix and why it exists (Purpose, Settings, "The two processes");
dropping bans once SWWAF_MAX_BANS is reached ("Counting method", "Bans", the SWWAF_MAX_BANS setting);
port 8080 forwarding to 127.0.0.1:8081 (Architecture, "Admin endpoints", "Deployment", "Ports");
a token under 32 characters stopping the start (Core, Metrics, "Admin endpoints");
the 431 answer to oversized headers (twice in "Size and time limits");
docker not showing that a single mounted file was replaced (lookup, "Rule files");
the CrowdSec ban (step 7, "Bans", "Reputation");
an attack ban turning permanent (step 4, "Bans", the ban action, Risks);
what the Core Rule Set reads, and the body limit (step 12, the "Attack detection" intro, SWWAF_WAF_BODY_LIMIT);
what counts toward the error burst (step 15, SWWAF_ERROR_BURST_THRESHOLD);
the lowest percentage applying (step 8, "Biased thresholds");
putting Actions runners in SWWAF_RATE_LIMIT_EXEMPT_NETS (the rate limits intro, the gitea notes);
GeoJS failing while the exclusive country list is set (lookup, "Failure behaviour");
no database and no disk reads while serving (Architecture, "Persistent state");
rules 930130 and 920440 being switched off (SWWAF_WAF_DISABLED_RULES, "Rule files");
the bans.json write delay (the setting, "Writing");
"No setting is required" (Settings, "Deployment").
Acceptable: each requirement stated once where it belongs, and other places link to it. The roughly 1,500 words of "Notes specific to gitea" are observations, not requirements, as the PR body itself says. They go to README.md or docs/, unless the owner rules that they stay.
2. Broken list at 1542. "For such a file, these requests are refused: - the API's list of its commits (path); - …" renders as one paragraph with literal dashes. Acceptable: a real nested list.
3. Non-goal and planned item contradict each other. Non-goals lists "Sharing bans between the instances of several apps.", and "Planned, not built" lists the same thing as planned. Acceptable: it appears in one place only.
4. The off rule at 268–275 is wrong against the code. This is the first difference the PR body lists. The code also refuses off for SWWAF_MAX_TRACKED_CLIENTS, SWWAF_MAX_BANS, SWWAF_ABUSEIPDB_DAILY_BUDGET, SWWAF_LOG_REMOTE_BUFFER and SWWAF_METRICS_TOP_N. Those are limits, yet the text says only settings that are not limits refuse off. The code also refuses it for the percentage settings, such as SWWAF_ABUSEIPDB_MIN_SCORE. The text says off switches off "a timeout", but the lookup and reputation timeouts refuse it. Acceptable: a bulleted list of exactly the settings that take off, a line saying every other setting refuses it, and the PR body item corrected.
5. Step 7 is false. "A cached DNS blocklist verdict or AbuseIPDB score gets SWWAF_REPUTATION_ACTION." Only a client a DNS blocklist lists, or one scored at or above SWWAF_ABUSEIPDB_MIN_SCORE, gets it. Acceptable: "A client listed by a DNS blocklist, or scored at least SWWAF_ABUSEIPDB_MIN_SCORE by AbuseIPDB, gets SWWAF_REPUTATION_ACTION."
6. Changes from the old spec that the PR body does not list.
Added: the SWWAF_IPV6_GROUP_PREFIX range of 32 to 128; off switching off the alert cooldown; the trap path in ban notes; refusals as a kind of limit hit, and dropped alerts, in the metrics; the summary event in the webhook schema.
Dropped: the daily disk-write figures for bans.json and lookups.json; the list of requests script/gitea-requests sends; that the app can read a token given as an environment variable, which is the reason for the file form.
#156 asks for every difference to be listed rather than settled silently. Acceptable: each one listed in the PR body, or restored.
7. Dangling references. The build order is gone and "Configuration surface" has been renamed, but other files still point to them:
README.md points to "the build order in SPEC.md" at about lines 69 and 1983, and in its TODO.
The package comments in internal/waf/waf.go and internal/anomaly/anomaly.go point to sections "under "Configuration surface"".
Acceptable: these references updated in this PR.
8. PR body length. It has about 390 words; the limit is about 250. Acceptable: a one-sentence intro and one short line per difference.
9. Failing sentences.
Over about 30 words, or a list chained with commas where bullets belong:
107: "With GeoJS, a request waits for its client's first answer, up to SWWAF_LOOKUP_TIMEOUT, only when a setting needs it before the request goes on: …". This also puts the exception before the main rule. Fix: "The request goes on at once. It waits for the first GeoJS answer, up to SWWAF_LOOKUP_TIMEOUT, only when one of these needs it:", followed by a list.
324: "SWWAF_STATE_WRITE_DELAY (default 10s): after a ban is made, lifted or made permanent, bans.json is written this long later, with every change made in between, so a burst of changes becomes one write." The subject comes late. Fix: "bans.json is written this long after a ban is made, lifted or made permanent, so a burst of changes becomes one write."
392: "There is one size limit for request bodies and one for response bodies: smallwebwaf passes bodies through unchanged, so a limit on each side would bound the same bytes, and the lower one would always decide." Split it.
403: "It must be more than 4K and cannot be off: Go's HTTP server always has such a limit, and reads 4 KiB past the one it is given before it refuses." Split it.
432: "While a request body is still on its way, a request timeout that runs out answers 408 if smallwebwaf was waiting for the client to send more at that moment." Split it, subject first.
645: "Ten query parameters are not checked against the Core Rule Set's lists of system files (930120), shell paths (932160) and command names (932260): path, files, … and redirect_to." Put the ten names in a bulleted list.
670: "In Referer it takes the address of a search for one word, such as env, set or last, when the address ends in it, and the address of OpenJDK's …/Runtime.java." Split it, and restore the old reason: the path holds both runtime and java..
718: "Spamhaus's terms for it, on its DROP page: automated downloads must be at least one hour apart, …; a product that uses it must credit The Spamhaus Project and keep the list's date and copyright lines with the data." Use bullets.
766: "file_error is a rule file or state file edited while running that does not parse, a replacement lookup database that cannot be read, or a state file that cannot be written." Use bullets.
893: "The Core Rule Set's rules for file names and extensions (930130, 920440) refuse such names anywhere in a path, which is why the default SWWAF_WAF_DISABLED_RULES switches them off: …". It is too long and repeats 577–592. Drop it.
918: "Per entry: the netblock, start, expiry …, a short reason, when it was lifted if an admin lifted it, and the notes below." Use bullets, as clients.json below does.
940: "alerts.json: the anomaly counters; for each event and netblock, source, file or anomaly scope, …; the alerts sent this hour; and the alerts still waiting to be sent." Use bullets.
950: "what was broken: the rule id and target that matched, or the trap path, or the limit, its window, the count reached and the client's limit percentage with what set it; and any reputation sources that listed the client;" Split it.
974: "Format: indented JSON with a top-level version number, entries sorted by client address, times in RFC 3339 UTC, …". Use bullets.
1022: "A write that fails while running: state stays correct in memory, the failure is logged, counted in the metrics, …, and retried at the next write." Use bullets.
1027: "What a hard kill can lose: …". Use bullets.
1100, 1107 and 1113: the "Limits and bans", "Lookup and reputation" and "Housekeeping" metric bullets, each 35 to 60 words chained with semicolons. Use nested bullets.
1229: "Its run script waits one second (sleep 1) and then starts the app with chpst, listening on 127.0.0.1:8081, as a user of its own that the Dockerfile creates with useradd." Split it.
1297: "So this one install uses those of the Go image that smallwebwaf is built in, which is pinned by digest too: apt's Acquire::https::CaInfo option names …, mounted for that one step." Split it.
1378: "The address part of SWWAF_LISTEN_ADDR stays empty (…), so smallwebwaf keeps listening on every address: traefik reaches it on the container's address, and the health check on 127.0.0.1." Split it.
1445: "Additions to consider at any time: an alert destination, country lists or biased thresholds, …, and an app-specific rule file." Use bullets.
1521: "A release started from such a tag, an issue from such a template, or such a rule opened for editing gets the 403 answer of smallwebwaf in place of its form." Split it.
1555: "The same for a path that holds what it takes for Node.js code (934100), as process.env.d.ts does, and for a path with a directory named ... …". Split it.
1562: "That is an image whose owner, or whose name after the owner, starts with an entry on the list, such as …". Split it.
1567: "A docker that keeps its images in containerd's image store, as a new install of Docker Engine 29 does, then fails the push, and fails each new try the same way." Split it.
1585: "So it passes the day limit of 50,000 after about an hour and a quarter of jobs: it is banned, the job it is running fails, and each repeat within a day makes the ban longer." Split it.
1616: "In short: the only things that stop the process happen at start: invalid configuration, …, and an unwritable SWWAF_STATE_DIR." Use bullets.
1655: "An app that uses one of the query parameters left out of 930120, 932160 and 932260 as a file on the server, …: no setting restores those rules." The main clause comes last, after 40 words. Lead with it.
Stacked clauses, a late verb or object, or a sentence a newcomer has to read twice:
125 and 236: "A client the CrowdSec decision list lists is banned until the decision ends." Fix: "A client on the CrowdSec decision list is banned until the decision ends."
247: "So do clients a reputation source lists whose action is …". Fix: "So do clients listed by a reputation source whose action is …".
156: "The response bytes, the request body bytes, or both are added to the byte counters." It does not say what decides. Fix: "The bytes SWWAF_BYTES_COUNT names are added to the byte counters."
181: "When SWWAF_MAX_BANS bans smallwebwaf made are held, the one that has gone longest without a request is dropped first, past, active or permanent." Fix: "When smallwebwaf holds SWWAF_MAX_BANS bans of its own, it drops the one that has gone longest without a request, whether past, active or permanent."
189: "It ends when an admin lifts it, or, if smallwebwaf made it, when it is dropped past SWWAF_MAX_BANS." "Dropped past" is unclear. The whole first paragraph of "Bans" is about the case where the table of bans is full, an edge case placed before the main rules. Fix: open "Bans" with what a ban is and the two rules, and state the full-table case once, after them.
277: "Every setting may instead be given as a file holding the value, named by the setting's name with _FILE added, such as SWWAF_ADMIN_TOKEN_FILE, for secrets and long lists." It reads as if the file has that name. Fix: "Any setting can be read from a file instead, for secrets and long lists: SWWAF_ADMIN_TOKEN_FILE names the file that holds SWWAF_ADMIN_TOKEN."
302: "Set it, for example to fsn1app1/gitea, to a name that stays the same when a deploy replaces the container and tells instances apart." Fix: "Set it to a name that survives a redeploy and tells instances apart, such as fsn1app1/gitea."
602: "In front of gitea it refuses, until the rule ids the request log names are added to SWWAF_WAF_DISABLED_RULES:". The object only arrives in the list. Fix: "In front of gitea it refuses the requests below, until their rule ids, named in the request log, are added to SWWAF_WAF_DISABLED_RULES:".
648: "gitea sends file paths, branch and workflow names, and the page to return to after signing in, in them." Fix: "gitea puts file paths, branch and workflow names, and the page to return to after signing in into them."
653: "What only these three rules refuse is let through there. That is more than a name such as /etc/passwd or whoami on its own: …". Fix: "In these parameters, anything only these three rules catch reaches the app, commands included: …".
666: "gitea writes into gitea_flash a message for the next page naming what was just done, and into redirect_to the page to return to after signing in." Fix: one sentence per cookie, subject, verb, then object.
668: "In these cookies the Core Rule Set takes names such as …, and titles that hold them, for attacks." Fix: "The Core Rule Set takes some names in these cookies for attacks, such as …".
707: "A source refuses the clients that a list kept elsewhere names, or lowers their limits, while the defaults judge a client only by what it does to this service." Fix: "A source acts on a list kept elsewhere; the defaults judge a client only by what it does here."
1173: "An app is hardened just by changing its base image, and perhaps installing on top of it, from nixpkgs, the packages it needs." Fix: "An app is hardened by changing its base image. It can add packages from nixpkgs."
1488: "The lists refuse such a name in any other query parameter too, …". "The lists" has nothing to refer to yet.
1523: "Saving such a rule stores it, but the branch settings page gitea then returns to gets the 403 answer, so the save looks as if it failed." Fix: "Saving such a rule stores it. gitea then returns to the branch settings page, which gets the 403 answer, so the save looks as if it failed."
1528: "The parameters the fourth change leaves out of 930120, 932160 and 932260 are included." The clauses are stacked, and the changes are not numbered. Fix: name the parameters, or link to the section.
Project history in a document for users:
1172: "Each app remains one container, deployed as before."
1424: "which now starts FROM the smallwebwaf image".
1438: "and deploy it as before".
Fix: drop "as before" and "now".
Judgement call: whether the gitea notes stay in the spec is left to the owner rather than failed outright.

- 1567: "A docker that keeps its images in containerd's image store, as a new install of Docker Engine 29 does, then fails the push, and fails each new try the same way." Split it.
- 1585: "So it passes the day limit of 50,000 after about an hour and a quarter of jobs: it is banned, the job it is running fails, and each repeat within a day makes the ban longer." Split it.
- 1616: "In short: the only things that stop the process happen at start: invalid configuration, …, and an unwritable `SWWAF_STATE_DIR`." Use bullets.
- 1655: "An app that uses one of the query parameters left out of 930120, 932160 and 932260 as a file on the server, …: no setting restores those rules." The main clause comes last, after 40 words. Lead with it.
Stacked clauses, a late verb or object, or a sentence a newcomer has to read twice:
- 125 and 236: "A client the CrowdSec decision list lists is banned until the decision ends." Fix: "A client on the CrowdSec decision list is banned until the decision ends."
- 247: "So do clients a reputation source lists whose action is …". Fix: "So do clients listed by a reputation source whose action is …".
- 156: "The response bytes, the request body bytes, or both are added to the byte counters." It does not say what decides. Fix: "The bytes `SWWAF_BYTES_COUNT` names are added to the byte counters."
- 181: "When `SWWAF_MAX_BANS` bans `smallwebwaf` made are held, the one that has gone longest without a request is dropped first, past, active or permanent." Fix: "When `smallwebwaf` holds `SWWAF_MAX_BANS` bans of its own, it drops the one that has gone longest without a request, whether past, active or permanent."
- 189: "It ends when an admin lifts it, or, if `smallwebwaf` made it, when it is dropped past `SWWAF_MAX_BANS`." "Dropped past" is unclear. The whole first paragraph of "Bans" is about the case where the table of bans is full, an edge case placed before the main rules. Fix: open "Bans" with what a ban is and the two rules, and state the full-table case once, after them.
- 277: "Every setting may instead be given as a file holding the value, named by the setting's name with `_FILE` added, such as `SWWAF_ADMIN_TOKEN_FILE`, for secrets and long lists." It reads as if the file has that name. Fix: "Any setting can be read from a file instead, for secrets and long lists: `SWWAF_ADMIN_TOKEN_FILE` names the file that holds `SWWAF_ADMIN_TOKEN`."
- 302: "Set it, for example to `fsn1app1/gitea`, to a name that stays the same when a deploy replaces the container and tells instances apart." Fix: "Set it to a name that survives a redeploy and tells instances apart, such as `fsn1app1/gitea`."
- 602: "In front of gitea it refuses, until the rule ids the request log names are added to `SWWAF_WAF_DISABLED_RULES`:". The object only arrives in the list. Fix: "In front of gitea it refuses the requests below, until their rule ids, named in the request log, are added to `SWWAF_WAF_DISABLED_RULES`:".
- 648: "gitea sends file paths, branch and workflow names, and the page to return to after signing in, in them." Fix: "gitea puts file paths, branch and workflow names, and the page to return to after signing in into them."
- 653: "What only these three rules refuse is let through there. That is more than a name such as `/etc/passwd` or `whoami` on its own: …". Fix: "In these parameters, anything only these three rules catch reaches the app, commands included: …".
- 666: "gitea writes into `gitea_flash` a message for the next page naming what was just done, and into `redirect_to` the page to return to after signing in." Fix: one sentence per cookie, subject, verb, then object.
- 668: "In these cookies the Core Rule Set takes names such as …, and titles that hold them, for attacks." Fix: "The Core Rule Set takes some names in these cookies for attacks, such as …".
- 707: "A source refuses the clients that a list kept elsewhere names, or lowers their limits, while the defaults judge a client only by what it does to this service." Fix: "A source acts on a list kept elsewhere; the defaults judge a client only by what it does here."
- 1173: "An app is hardened just by changing its base image, and perhaps installing on top of it, from nixpkgs, the packages it needs." Fix: "An app is hardened by changing its base image. It can add packages from nixpkgs."
- 1488: "The lists refuse such a name in any other query parameter too, …". "The lists" has nothing to refer to yet.
- 1523: "Saving such a rule stores it, but the branch settings page gitea then returns to gets the `403` answer, so the save looks as if it failed." Fix: "Saving such a rule stores it. gitea then returns to the branch settings page, which gets the `403` answer, so the save looks as if it failed."
- 1528: "The parameters the fourth change leaves out of 930120, 932160 and 932260 are included." The clauses are stacked, and the changes are not numbered. Fix: name the parameters, or link to the section.
Project history in a document for users:
- 1172: "Each app remains one container, deployed as before."
- 1424: "which now starts `FROM` the `smallwebwaf` image".
- 1438: "and deploy it as before".
Fix: drop "as before" and "now".
Judgement call: whether the gitea notes stay in the spec is left to the owner rather than failed outright.
Model: opus-5-5
Every requirement is stated once, where it belongs, and linked from
elsewhere. The notes on gitea move to docs/gitea.md, which
script/gitea-requests points at. The list of settings that take off
is exact, the non-goal that contradicted the planned item is gone,
and the sentences the review named are split or reordered. The
package comments in internal/waf and internal/anomaly point at the
renamed sections.
Model: fable-5-1
Reworked in e44ba7e. Every finding is applied: each requirement is stated once, with links from elsewhere (SPEC.md is about 11,700 words, from 15,100; the gitea notes, 1,700 words, are in docs/gitea.md); the nested list, the non-goal, the exact off list, step 7, the unlisted differences, the PR body (under 250 words) and every sentence named. One rebuttal, on the README.md part of finding 7: #157 rewrites README.md and has no reference to the build order, so this PR leaves README.md alone and the two branches do not conflict. The package comments in internal/waf/waf.go and internal/anomaly/anomaly.go are updated here, and script/gitea-requests points at docs/gitea.md. make fmt applied; script/cibuild green.

Model: fable-5-1
Line numbers are those of the new SPEC.md. Every finding of the first review is fixed, apart from the README.md part of finding 7, which is fairly answered by #157 (see the disclosure at the end).
1. Requirements still said more than once.#156 asks for each requirement once. These still appear in two to five places:
the ban lengths and their defaults: "Bans" (166–181) and "Ban lengths and response" (626–633);
responses not inspected: 548 and the sixth change (618);
the bans.json write delay: the setting (291) and "Writing" (927);
a country-list refusal is neither an offence nor a ban: step 6 (94), "Bans" (196), "Country lists" (473);
a client the lookup cannot place: the lookup (434), "Country lists" (461), SWWAF_UNKNOWN_LIMIT_PERCENT (491);
a lower percentage bans sooner: 484, 498, 666 and 685;
a Core Rule Set match refuses only that request, without a ban: 186, the block action (776), Risks (1360);
ban path rules anchored at the site root: 779, 828, Risks (1353);
token guessing banned through the error burst: 1099 and Risks (1372);
/_smallwebwaf/ never reaching the app: 48, step 13 (110), 1074;
adding or lifting a ban by editing bans.json: 201, 275, 953, 1086, 1102, 1327;
what the image holds: Architecture (41–45) and "The image holds" (1124–1151);
the Core Rule Set changes hold in front of every app and no setting undoes them: 572 and 1366;
about 15 MiB of GeoJS answers: 438 and 902.
Acceptable: each stated once where it belongs, with a link from anywhere else that needs it.
2. Sections that add no requirement of their own.
"Risks the design has to handle" (1340–1380, about 430 words) repeats rules stated elsewhere. Only two points are not stated elsewhere: an engine error forwards the request, and per-client limits miss widely spread scrapers. Move those two into "Attack detection" and "Anomaly thresholds", and drop the section.
"Ban lengths and response" (624–639) repeats "Bans". Keep SWWAF_BAN_RESPONSE and SWWAF_BAN_SCOPE_V4_PREFIX. Give the four length settings as names that link to "Bans".
Architecture (39–64) repeats the opening and "Deployment". "The other parts of the process are each specified below." says nothing. Keep the library list and the "no database" rule.
"Rollout per service" (1320–1338) is advice to the operator, which belongs in README.md.
The list of what body inspection refuses in front of gitea (549–556) is an observation and belongs in docs/gitea.md.
3. The off rule is false (233). "Every other setting refuses off." SWWAF_LOOKUP_SOURCE and SWWAF_WAF_MODE both take off. Acceptable: add both to the list.
4. Missing bound (141–142). "The GeoJS answers, the reputation verdicts and the anomaly counters have fixed bounds, given where each is described." No bound for reputation verdicts is given anywhere. Acceptable: under "Reputation", say "Up to 100,000 DNS blocklist verdicts and 100,000 AbuseIPDB scores are kept. Past that, the one fetched longest ago goes first."
5. False on its own (206). "It ends when an admin lifts it, or when the table of bans is full." A full table does not end every permanent ban. Acceptable: "It ends when an admin lifts it, or when smallwebwaf drops it from a full table, as below."
6. Misleading (681). "its decision list is pulled on a schedule and treated as a blocklist". That reads as if SWWAF_BLOCKLIST_ACTION applies. Acceptable: "It is kept like a fetched blocklist, and a client on it is banned (see "Bans")."
7. Requirements dropped without a note in the PR body. Acceptable for each: restore it in one short sentence, or list it in the PR body.
794: the log says when no rules were loaded.
108 and 523: what counts as a Core Rule Set match, namely an anomaly score at or above SWWAF_WAF_ANOMALY_THRESHOLD. Also missing: with off, nothing is refused.
Step 5: a request that ends before the GeoJS answer comes is logged without it.
59: the reason the changes are checked again: rule ids, and what each rule matches, change between versions.
355: the reason for one body limit per direction. A limit on each side would bound the same bytes, and the lower one would always decide. "since smallwebwaf passes bodies through unchanged" does not say this.
612: "and on every link followed from it", and the consequence that all those requests would be refused. Without them, the reason for the Referer change is incomplete.
8. Unnumbered list referred to by number (621, 1365). "The fourth and fifth changes": the six changes are a bulleted list. Acceptable: a numbered list.
9. Undefined terms.
87: "The ban ledger". Its definition went with the old component list. Acceptable: "Bans."
708 and 877: "anomaly scope". The spec never defines a scope. Acceptable: "or, for an anomaly, the same thing counted: one client, netblock, AS number or named netblock, or the whole service".
465: "a failing helper", and 1244: "a broken helper". Acceptable: name them, "GeoJS, a reputation source or an alert destination".
10. Failing sentences.
871: "reputation.json: the last good copy of each list fetched from a URL; the cached DNS blocklist verdicts …; and the AbuseIPDB checks spent today, so that a restart does not reset the daily budget." It runs to 43 words chained with semicolons. Use bullets, as alerts.json does.
972: "everything else from the last SWWAF_STATE_COUNTER_INTERVAL (15 minutes): counts and history, the counts in ban notes, GeoJS answers, …". Six items chained with commas. Use a nested list.
604–617: one bullet mixes the cookies and Referer. "gitea shows the message with any script removed and returns only to its own pages." comes after the Referer sentences but is about the cookies. Use two bullets, each with its own reason.
897: "clients.json takes about 1 KiB per client, about 20 MiB at the default SWWAF_MAX_TRACKED_CLIENTS, which written every 15 minutes is under 2 GiB of disk writes a day." Fix: end the sentence at the setting, then "Written every 15 minutes, that is under 2 GiB of disk writes a day."
437: "Each GeoJS answer is kept for 7 days, in memory and in lookups.json, apart from the table of clients." "Apart from" can mean "except". Fix: "kept separately from the table of clients, so it outlasts a client dropped from that table."
464–466: "That is the nearest a failing helper comes to taking the service down: GeoJS failing while this list is set refuses every new client." "That" points back to the opposite case. Fix: "The cost: while GeoJS fails, this list refuses every new client. No other failure comes this close to taking the service down."
146: "A ban refuses every request from a netblock with SWWAF_BAN_RESPONSE until it ends." The phrase "with SWWAF_BAN_RESPONSE" reads as part of the netblock. Fix: "Until a ban ends, every request from its netblock is refused with SWWAF_BAN_RESPONSE."
196: "SWWAF_DENY_NETS, a blocklist or reputation hit whose action is deny, and the country lists refuse each request they cover …". The verb comes late. Fix: "These refuse each request they cover and record nothing in bans.json:", followed by a list.
407: "The file is the alternative for a service that keeps its visitors' addresses on its own host." No file has been named yet. Fix: "A local database file is the alternative …".
416: "SWWAF_LOOKUP_DB_PATH: the file for file: the IPinfo Lite database …". Two colons in a row. Fix: "the database the file source reads: the IPinfo Lite database …".
410: "One source at a time: file without SWWAF_LOOKUP_DB_PATH, or SWWAF_LOOKUP_DB_PATH with any other source, stops the start with a message naming both." The verb comes late, and the next sentence chains four settings. Fix: "Only one source is used. The start stops, naming both settings, when …", followed by a list of the four settings.
440: "how long a request waits for its client's first answer when a setting needs it, and how long a request to GeoJS may take." "A request" means two different things. Split it: a visitor's request in one sentence, the request to GeoJS in the next.
282: "so it can read a token given as one." Fix: "so it can read a token set as an environment variable."
1087: "The client's current counters and history, which holds its lookup result and offences, and its bans with their notes." Fix: "It answers with the client's current counters, its history (with its lookup result and offences), and its bans with their notes."
1132–1134: "… lets root build without the build users nix-bin does not create." Fix: "nix-bin creates no build users, so build-users-group = in /etc/nix/nix.conf lets root build without them."
Judgement call: I accept the rebuttal on the README.md part of the first review's finding 7. Until #157 lands, README.md on next still points at the build order and the gitea notes in SPEC.md. That PR should land before this one or with it.

Model: opus-5-5
Every requirement the second review found in two to five places is now
stated once, with links from the other places. "Risks the design has to
handle", "Rollout per service" and most of "Architecture" are gone, their
two points of their own moved to "Attack detection" and "Anomaly
thresholds". The request log's fields, the metrics and the image's
contents move to docs/request-log.md, docs/metrics.md and docs/image.md,
each linked from the spec. The off rule names SWWAF_LOOKUP_SOURCE and
SWWAF_WAF_MODE, the reputation bound is given, and the six Core Rule Set
changes are a numbered list.
Model: fable-5-1
Reworked in eff99f0. Every finding of the second review is applied, none rebutted. Each repeated requirement is stated once, with links from the other places. "Risks the design has to handle", "Rollout per service" and most of "Architecture" are gone; the two points of their own moved to "Attack detection" and "Anomaly thresholds". "Ban lengths and response" is now "Ban response and scope" and names the four length settings as links to "Bans". The gitea body-inspection list is in docs/gitea.md. The off list names SWWAF_LOOKUP_SOURCE and SWWAF_WAF_MODE; the reputation bound is given; the full-table and CrowdSec sentences are reworded; the six dropped requirements are restored; the six changes are a numbered list; the undefined terms are replaced; every sentence named is rewritten. The request log's fields, the metrics and the image's contents moved to docs/request-log.md, docs/metrics.md and docs/image.md, each linked from the spec, which keeps one sentence per requirement for them. SPEC.md is about 10,300 words, from 11,700; the four docs/ files hold about 3,500. The PR body is rewritten under 250 words.

"Ban lengths and response" is now "Ban response and scope" and names the four length settings as links to "Bans". The gitea body-inspection list is in `docs/gitea.md`. The `off` list names `SWWAF_LOOKUP_SOURCE` and `SWWAF_WAF_MODE`; the reputation bound is given; the full-table and CrowdSec sentences are reworded; the six dropped requirements are restored; the six changes are a numbered list; the undefined terms are replaced; every sentence named is rewritten. The request log's fields, the metrics and the image's contents moved to `docs/request-log.md`, `docs/metrics.md` and `docs/image.md`, each linked from the spec, which keeps one sentence per requirement for them. `SPEC.md` is about 10,300 words, from 11,700; the four `docs/` files hold about 3,500. The PR body is rewritten under 250 words.
Model: fable-5-1
docs/settings.md is now the one settings reference; the spec keeps the
conventions, the off list and each group's requirements and reasons, and
links to it for the per-setting entries. The endpoint list, the ban
notes, the state file contents, the rule file format and the alert
object likewise live in docs/ and are linked from the spec. The package
list from the README's Design section joins "Architecture". The nine
docs files are byte-identical to those on the README branch. Package
comments that named "Alert webhook schema", "Request log" and "Metrics
endpoint" for their field lists now name the docs files.

Reworked again in d712bb1, on the manager's decision: SPEC.md keeps requirements and reasons, and the reference material lives in docs/, every fact in one file and linked from the others. docs/settings.md is the one settings reference; the spec keeps the conventions, the off list and each group's requirements and reasons, and links to it for the per-setting entries. The endpoint list, the ban notes, the state file contents, the rule file format and the default file, and the alert object are in docs/admin-endpoints.md, docs/bans.md, docs/state-files.md, docs/rules.md and docs/alerts.md, beside request-log.md, metrics.md, image.md, lookup.md, reputation.md, core-rule-set.md and gitea.md. The nine new files are byte-identical to those on #157. The package list from the README's Design section is now under "Architecture", so no second design document exists. Package comments that named "Alert webhook schema", "Request log" and "Metrics endpoint" for their field lists now name the docs files.
SPEC.md is about 8,900 words, from 10,300; the thirteen docs/ files hold about 12,900, of which settings.md is 4,100. No sentence is over 30 words. make fmt applied; script/cibuild green.

Line numbers are those of the files on the PR head. Most findings of the second review (#159 (comment)) are fixed. Its finding 1 is not: many requirements are now stated in SPEC.md and again in a docs/ file. Three of its sentence fixes were not applied (items 4 and 7 below).
1. The same requirement in two files. The rework says every fact lives in one file. The docs files restate SPEC.md, often word for word:
docs/core-rule-set.md:
what the Core Rule Set reads (3–6; SPEC.md 502–505, 533–536 and change 6);
a score at the threshold is a match, refused under block and let through under detect (11–13; SPEC.md 146–148, 515–516);
"An engine error on a request forwards the request, and is logged and counted." (18–19; SPEC.md 551–552);
"Body inspection suits apps whose forms carry no code." (21; SPEC.md 538);
what it refuses on a code forge (21–23; SPEC.md 505–508 and docs/gitea.md 181–182);
other bodies reach the app uninspected (27–28; SPEC.md 536–538).
docs/lookup.md:
GeoJS is asked whether or not a setting uses the answer (43–45; SPEC.md 430–431);
private addresses are never sent (49; SPEC.md 432–433);
batches, one request at a time (52–53; SPEC.md 433–434);
answers are kept separately from the table of clients (55–57; SPEC.md 435–436);
when a request waits, with the same list (60–69; SPEC.md step 5 and 440–443);
a late answer counts as unknown (70–72; SPEC.md 442–443);
GeoJS failing under the exclusive country list (72–73; SPEC.md 445–447, 466);
the operator downloads and mounts the database, which is never fetched (83–84; SPEC.md 421–422);
an unreadable replacement, and mounting the directory (90–93; SPEC.md 450–453, 727–729);
all of "A client without a country" (101–110; SPEC.md 437–439, word for word);
the 100,000 answers (57–58; "Fixed limits" in docs/settings.md).
docs/reputation.md:
the CrowdSec ban paragraph (46–50; SPEC.md 230–233, word for word);
the list kept as a blocklist is (41–42; SPEC.md 655);
the copy kept with its comment lines (28; SPEC.md 638–639);
the request goes on at once (63–64; SPEC.md step 7);
the definition of an offence (89–93; SPEC.md 192–197);
a score at or over the minimum is a hit (99–100; SPEC.md step 7);
the verdict and score bounds and the 1,000 queries (78–79, 101; "Fixed limits" in docs/settings.md).
docs/request-log.md:
the "type":"process" sentence (107–108; SPEC.md 783–784 word for word, and 333–335);
syslog sending, the buffer and dropping the oldest line (112–127; SPEC.md 339–341 and 786–791).
docs/image.md:
the list of contents (9–34; SPEC.md 831–839);
the run script giving the state directory to the smallwebwaf user (27–30; SPEC.md 924–925);
the same packages on each build (53; SPEC.md 841–842).
docs/gitea.md:
runners in SWWAF_RATE_LIMIT_EXEMPT_NETS, and what still applies to them (34–36; SPEC.md 352–353, 364–365). The first review named this one too.
the 413, 408 and 504 answers to uploads (19–22; SPEC.md 399–410);
a refusal answers only that request and counts toward the error burst (58–59; SPEC.md 225–228);
large uploads (14–24; docs/settings.md 94–98).
The other docs files do the same. Examples:
docs/state-files.md 30–31 repeats SPEC.md 749–750 word for word, and its "Writing" and "Editing while it runs" sections repeat SPEC.md 754–773;
docs/rules.md 31–46 and 106–113 repeat SPEC.md 707–729;
docs/bans.md 49–59 repeats SPEC.md 245–257 and 765–769;
the conventions in docs/settings.md (15–30) repeat SPEC.md 263–289.
Acceptable: each rule and fact in one file only. SPEC.md keeps the requirement and its reason. A docs file keeps only the detail it adds, and links to the SPEC.md section for the rest.
2. The settings groups do not match (SPEC.md 291–292). The text says "docs/settings.md lists every setting … in the groups below.", but that file's groups have other names and other contents. One concept also has three names: "Biased thresholds" in SPEC.md, "Lower limits by AS number and country" in docs/settings.md, and "the lower limits" in docs/lookup.md 66 and 109. Acceptable: the same group names in both files, and one name for each concept.
3. False or misleading statements.
SPEC.md 278: "Every other setting refuses off." Free-text settings take off as an ordinary value, for example SWWAF_INSTANCE_NAME, SWWAF_LOG_REQUEST_HEADERS and SWWAF_RULES_DIR. Acceptable: "No other setting can be switched off with off."
SPEC.md 313–315: "SWWAF_METRICS_TOKEN switches on the metrics, which would otherwise be open to anyone on the internet." This reads as if the metrics are open without the token, but without it they answer 404. Acceptable: "The metrics are off until SWWAF_METRICS_TOKEN is set, since without a token they would be open to anyone on the internet."
SPEC.md 351: "SWWAF_ALLOW_NETS bypasses everything". Step 3 says such a client is still counted for the anomaly thresholds and still needs a token. The size and time limits still apply to it too. Acceptable: "SWWAF_ALLOW_NETS skips the checks step 3 names, for monitoring or the owner's own networks."
SPEC.md 179–181 says the fixed bounds are under "Fixed limits" in docs/settings.md. The anomaly counters' bound is not there; SPEC.md 693 and docs/settings.md 443 both give it. Acceptable: the bound in one place, and a pointer that is correct.
SPEC.md 751: "Each file is rewritten whole, so each has a bound given by its setting." Cause and effect are reversed. Also, fixed limits, not settings, bound lookups.json, reputation.json and alerts.json. In 752, "these sizes" points at sizes now given only in docs/state-files.md. Acceptable: "Each file has a bound, so it can be rewritten whole."
docs/reputation.md 6–8: "…a source the client is on refuses it, lowers its limits, or only notes it, as SWWAF_BLOCKLIST_ACTION and SWWAF_REPUTATION_ACTION say." The CrowdSec decision list bans instead, and the sentence runs past 30 words. Acceptable: "A blocklist acts as SWWAF_BLOCKLIST_ACTION says. A DNS blocklist or AbuseIPDB acts as SWWAF_REPUTATION_ACTION says. The CrowdSec decision list bans."
4. Requirements said twice within SPEC.md.
the bytes SWWAF_BYTES_COUNT counts: 154–155 and 372–373;
remote log sending and its buffer: 339–341 and 786–791;
"type":"process": 333–335 and 783–784;
a client in SWWAF_ALLOW_NETS still needs a token: 120 and 823;
fetching the DROP list at most once an hour: 625–626 and 633;
the state directory beside the app's data, belonging to the smallwebwaf user: 325–326 and 920–925;
the exclusive list refusing a client the lookup cannot place: 437–439 and 462–464. The second review named this one.
SWWAF_WAF_BODY_LIMIT switching body inspection on: 510 and 533;
the database file as the choice for a service that keeps addresses on its own host: 419–421 and 448–449.
Acceptable: each said once, with a link from the other place.
5. Requirements dropped without a note in the PR body. Acceptable for each: restore it in one short sentence, or list it in the PR body.
the state directory is synced after the rename (the code does this);
active bans are held in an in-memory prefix lookup;
the lookup database is mounted read-only;
after a late GeoJS answer, the address is asked about again in the background;
why the Ubuntu snapshot is never older than the Ubuntu image: older packages can need older versions of packages the image holds;
that the snapshot moves with the image's digest;
why ca-certificates is installed by name;
files an earlier owner left in the state volume can be read and replaced;
X-Client-ASN and X-Client-Country are for the app's own logs.
6. Stale references in code.internal/state/state_test.go 1023 cites "Edits while running" in SPEC.md, which no longer exists. internal/state/state.go 2 says the "Persistent state" section describes what each file holds; that list is now in docs/state-files.md. Acceptable: both point at docs/state-files.md.
7. Failing sentences.
Over about 30 words, or a list chained with commas:
SPEC.md 7–9, in the opening: "The reference documents under docs/, which this document links to, give the settings, the formats and the lists; every fact lives in one of these files and is linked from the others." The verb also comes late. Fix: "The reference documents under docs/ give the settings, the formats and the lists. This document links to them."
SPEC.md 305–307: "Where other containers reach smallwebwaf directly, …; the setting, narrowed to traefik's own network, closes that gap." Fix: end the sentence at "header", then "Narrowing the setting to traefik's own network closes that gap."
SPEC.md 407–410: "A request timeout that runs out while the body is still on its way answers 408 if … and 504 if it was waiting for the app." Fix: "A request timeout can run out while the body is on its way. The answer is then 408 if smallwebwaf was waiting for the client, and 504 if it was waiting for the app."
SPEC.md 419–422: "A local database file, the free IPinfo Lite database, is the alternative …; the operator downloads and mounts it, and smallwebwaf never fetches it and holds no account or token for it." Fix: end the sentence at "host", then "The operator downloads and mounts it. smallwebwaf never fetches it and holds no account or token for it."
SPEC.md 427–429: "…a setting needs lookups: a country list, a biased threshold, SWWAF_ADD_LOOKUP_HEADERS or an anomaly threshold per AS number." The second review asked for a list here. Fix: a bulleted list.
SPEC.md 508–510: "The default gives up refusing an attack carried in a body; the rule files, the limits and the bans still apply …, and SWWAF_WAF_BODY_LIMIT switches body inspection on." Fix: two sentences, without the last clause, which 533 states.
SPEC.md 533–536: "Form data and multipart bodies are read up to the size, the rest streaming on …; JSON and XML bodies are read only when no larger than the size, since those cannot be read in part." Fix: two sentences, the first ending "the rest streams on without being held in memory."
SPEC.md 684–686: "They are what reveals a widely distributed scraper, …; a low SWWAF_ASN_LIMIT_PERCENT for its AS number is the response." Fix: "They reveal a widely distributed scraper, thousands of addresses at low rates each, which the per-client limits do not see. A low SWWAF_ASN_LIMIT_PERCENT for its AS number is the response."
docs/request-log.md 69–71: "counts: the client's requests in the minute, hour and day …; its bytes as minute_bytes, hour_bytes and day_bytes." Fix: a nested list.
docs/gitea.md 16–17: "…every other upload: a git push, an LFS object, a container image layer, a package file or a release attachment." Fix: a nested list.
Hard to follow on a first reading:
SPEC.md 60–62: "picks the ban to drop past SWWAF_MAX_BANS". The first review called "dropped past" unclear. Fix: "picks the ban to drop when the table of bans is full."
SPEC.md 65–66: "the six changes" appears before anything has introduced them. Fix: "with the six changes listed under "Attack detection"".
SPEC.md 345–347: "…bounds the series by AS number and by country: that many of the busiest each, the rest summed as other, so the label set stays bounded." Fix: "SWWAF_METRICS_TOP_N is how many of the busiest AS numbers and countries get a series of their own. The rest are summed as other."
SPEC.md 440–442: "SWWAF_LOOKUP_TIMEOUT bounds both how long a visitor's request waits for its client's first answer, when a setting needs it, and how long a request to GeoJS may take." The second review asked for a split. Fix: "SWWAF_LOOKUP_TIMEOUT is how long a visitor's request waits for its client's first answer. It also bounds each request to GeoJS."
SPEC.md 448–449: "A service that cannot accept this, or that every new visitor's address goes to a third party, uses the file." The two halves do not fit together. Fix: "A service that cannot accept this uses the file."
SPEC.md 450–451: "…sent as one file_error alert, and ignored, the file in use staying in use." Fix: "One that cannot be read is logged, sent as one file_error alert and ignored. The database already loaded stays in use."
SPEC.md 544–546: "SWWAF_ERROR_BURST_THRESHOLD bounds the refusals per client per minute after a rule file match, …". "After" reads as a time. Fix: "…counts a client's refusals per minute for a rule file match, a trap path, a Core Rule Set match, or a missing or wrong token."
SPEC.md 646–647: "DNS blocklists (SWWAF_DNSBL_ZONES) meant for mail list residential ranges and would block ordinary visitors". The words "mail list" read as one noun. Fix: "DNS blocklists made for mail servers list residential ranges, and would block ordinary visitors."
SPEC.md 828: "Each app remains one container." "Remains" tells of a change from an earlier setup. Fix: "Each app is one container."
docs/reputation.md 95–96: "at the client's first request after the offence that no ban refuses". The phrase "that no ban refuses" seems to describe the offence. Fix: "The check runs in the background at the client's next request that no ban refuses, so no request waits."
docs/gitea.md 153–155: "The request is refused when the image's owner, or its name after the owner, starts with an entry on the list of commands." The first review asked for a split. Fix: "The request is refused when the image's owner starts with an entry on the list of commands. So is one whose name after the owner does."
Judgement call: some facts in docs/ were never in the old SPEC.md, such as the RFC 5782 query names, the 16 MiB fetch limit and the score for more than 1000 parameters. They come from README.md on next through #157. I treat them as moved README content, not additions to the spec.
Judgement call, carried from the second review: README.md on next still points at SPEC.md sections this PR removes. #157 must land with this one or before it.
Not reviewed sentence by sentence: docs/settings.md, docs/state-files.md, docs/rules.md, docs/bans.md, docs/admin-endpoints.md, docs/alerts.md and docs/metrics.md, which another reviewer covers. I read them only for coverage and for repeats of SPEC.md.
Model: opus-5-5
**FAIL: needs-rework**
Line numbers are those of the files on the PR head. Most findings of the second review (https://git.eeqj.de/sneak/smallwebwaf/pulls/159#issuecomment-154352) are fixed. Its finding 1 is not: many requirements are now stated in `SPEC.md` and again in a `docs/` file. Three of its sentence fixes were not applied (items 4 and 7 below).
**1. The same requirement in two files.** The rework says every fact lives in one file. The docs files restate `SPEC.md`, often word for word:
- `docs/core-rule-set.md`:
- what the Core Rule Set reads (3–6; `SPEC.md` 502–505, 533–536 and change 6);
- a score at the threshold is a match, refused under `block` and let through under `detect` (11–13; `SPEC.md` 146–148, 515–516);
- "An engine error on a request forwards the request, and is logged and counted." (18–19; `SPEC.md` 551–552);
- "Body inspection suits apps whose forms carry no code." (21; `SPEC.md` 538);
- what it refuses on a code forge (21–23; `SPEC.md` 505–508 and `docs/gitea.md` 181–182);
- other bodies reach the app uninspected (27–28; `SPEC.md` 536–538).
- `docs/lookup.md`:
- GeoJS is asked whether or not a setting uses the answer (43–45; `SPEC.md` 430–431);
- private addresses are never sent (49; `SPEC.md` 432–433);
- batches, one request at a time (52–53; `SPEC.md` 433–434);
- answers are kept separately from the table of clients (55–57; `SPEC.md` 435–436);
- when a request waits, with the same list (60–69; `SPEC.md` step 5 and 440–443);
- a late answer counts as unknown (70–72; `SPEC.md` 442–443);
- GeoJS failing under the exclusive country list (72–73; `SPEC.md` 445–447, 466);
- the operator downloads and mounts the database, which is never fetched (83–84; `SPEC.md` 421–422);
- an unreadable replacement, and mounting the directory (90–93; `SPEC.md` 450–453, 727–729);
- all of "A client without a country" (101–110; `SPEC.md` 437–439, word for word);
- the 100,000 answers (57–58; "Fixed limits" in `docs/settings.md`).
- `docs/reputation.md`:
- the CrowdSec ban paragraph (46–50; `SPEC.md` 230–233, word for word);
- the list kept as a blocklist is (41–42; `SPEC.md` 655);
- the copy kept with its comment lines (28; `SPEC.md` 638–639);
- the request goes on at once (63–64; `SPEC.md` step 7);
- the definition of an offence (89–93; `SPEC.md` 192–197);
- a score at or over the minimum is a hit (99–100; `SPEC.md` step 7);
- the verdict and score bounds and the 1,000 queries (78–79, 101; "Fixed limits" in `docs/settings.md`).
- `docs/request-log.md`:
- the `"type":"process"` sentence (107–108; `SPEC.md` 783–784 word for word, and 333–335);
- syslog sending, the buffer and dropping the oldest line (112–127; `SPEC.md` 339–341 and 786–791).
- `docs/image.md`:
- the list of contents (9–34; `SPEC.md` 831–839);
- the `run` script giving the state directory to the `smallwebwaf` user (27–30; `SPEC.md` 924–925);
- the same packages on each build (53; `SPEC.md` 841–842).
- `docs/gitea.md`:
- runners in `SWWAF_RATE_LIMIT_EXEMPT_NETS`, and what still applies to them (34–36; `SPEC.md` 352–353, 364–365). The first review named this one too.
- the `413`, `408` and `504` answers to uploads (19–22; `SPEC.md` 399–410);
- a refusal answers only that request and counts toward the error burst (58–59; `SPEC.md` 225–228);
- large uploads (14–24; `docs/settings.md` 94–98).
- The other docs files do the same. Examples:
- `docs/state-files.md` 30–31 repeats `SPEC.md` 749–750 word for word, and its "Writing" and "Editing while it runs" sections repeat `SPEC.md` 754–773;
- `docs/rules.md` 31–46 and 106–113 repeat `SPEC.md` 707–729;
- `docs/admin-endpoints.md` 3–10 repeats `SPEC.md` 813–823;
- `docs/bans.md` 49–59 repeats `SPEC.md` 245–257 and 765–769;
- the conventions in `docs/settings.md` (15–30) repeat `SPEC.md` 263–289.
Acceptable: each rule and fact in one file only. `SPEC.md` keeps the requirement and its reason. A docs file keeps only the detail it adds, and links to the `SPEC.md` section for the rest.
**2. The settings groups do not match (`SPEC.md` 291–292).** The text says "`docs/settings.md` lists every setting … in the groups below.", but that file's groups have other names and other contents. One concept also has three names: "Biased thresholds" in `SPEC.md`, "Lower limits by AS number and country" in `docs/settings.md`, and "the lower limits" in `docs/lookup.md` 66 and 109. Acceptable: the same group names in both files, and one name for each concept.
**3. False or misleading statements.**
- `SPEC.md` 278: "Every other setting refuses `off`." Free-text settings take `off` as an ordinary value, for example `SWWAF_INSTANCE_NAME`, `SWWAF_LOG_REQUEST_HEADERS` and `SWWAF_RULES_DIR`. Acceptable: "No other setting can be switched off with `off`."
- `SPEC.md` 313–315: "`SWWAF_METRICS_TOKEN` switches on the metrics, which would otherwise be open to anyone on the internet." This reads as if the metrics are open without the token, but without it they answer `404`. Acceptable: "The metrics are off until `SWWAF_METRICS_TOKEN` is set, since without a token they would be open to anyone on the internet."
- `SPEC.md` 351: "`SWWAF_ALLOW_NETS` bypasses everything". Step 3 says such a client is still counted for the anomaly thresholds and still needs a token. The size and time limits still apply to it too. Acceptable: "`SWWAF_ALLOW_NETS` skips the checks step 3 names, for monitoring or the owner's own networks."
- `SPEC.md` 179–181 says the fixed bounds are under "Fixed limits" in `docs/settings.md`. The anomaly counters' bound is not there; `SPEC.md` 693 and `docs/settings.md` 443 both give it. Acceptable: the bound in one place, and a pointer that is correct.
- `SPEC.md` 751: "Each file is rewritten whole, so each has a bound given by its setting." Cause and effect are reversed. Also, fixed limits, not settings, bound `lookups.json`, `reputation.json` and `alerts.json`. In 752, "these sizes" points at sizes now given only in `docs/state-files.md`. Acceptable: "Each file has a bound, so it can be rewritten whole."
- `docs/reputation.md` 6–8: "…a source the client is on refuses it, lowers its limits, or only notes it, as `SWWAF_BLOCKLIST_ACTION` and `SWWAF_REPUTATION_ACTION` say." The CrowdSec decision list bans instead, and the sentence runs past 30 words. Acceptable: "A blocklist acts as `SWWAF_BLOCKLIST_ACTION` says. A DNS blocklist or AbuseIPDB acts as `SWWAF_REPUTATION_ACTION` says. The CrowdSec decision list bans."
**4. Requirements said twice within `SPEC.md`.**
- the bytes `SWWAF_BYTES_COUNT` counts: 154–155 and 372–373;
- remote log sending and its buffer: 339–341 and 786–791;
- `"type":"process"`: 333–335 and 783–784;
- a client in `SWWAF_ALLOW_NETS` still needs a token: 120 and 823;
- fetching the DROP list at most once an hour: 625–626 and 633;
- the state directory beside the app's data, belonging to the `smallwebwaf` user: 325–326 and 920–925;
- the exclusive list refusing a client the lookup cannot place: 437–439 and 462–464. The second review named this one.
- `SWWAF_WAF_BODY_LIMIT` switching body inspection on: 510 and 533;
- the database file as the choice for a service that keeps addresses on its own host: 419–421 and 448–449.
Acceptable: each said once, with a link from the other place.
**5. Requirements dropped without a note in the PR body.** Acceptable for each: restore it in one short sentence, or list it in the PR body.
- the state directory is synced after the rename (the code does this);
- active bans are held in an in-memory prefix lookup;
- the lookup database is mounted read-only;
- after a late GeoJS answer, the address is asked about again in the background;
- why the Ubuntu snapshot is never older than the Ubuntu image: older packages can need older versions of packages the image holds;
- that the snapshot moves with the image's digest;
- why `ca-certificates` is installed by name;
- files an earlier owner left in the state volume can be read and replaced;
- `X-Client-ASN` and `X-Client-Country` are for the app's own logs.
**6. Stale references in code.** `internal/state/state_test.go` 1023 cites "Edits while running" in `SPEC.md`, which no longer exists. `internal/state/state.go` 2 says the "Persistent state" section describes what each file holds; that list is now in `docs/state-files.md`. Acceptable: both point at `docs/state-files.md`.
**7. Failing sentences.**
Over about 30 words, or a list chained with commas:
- `SPEC.md` 7–9, in the opening: "The reference documents under `docs/`, which this document links to, give the settings, the formats and the lists; every fact lives in one of these files and is linked from the others." The verb also comes late. Fix: "The reference documents under `docs/` give the settings, the formats and the lists. This document links to them."
- `SPEC.md` 305–307: "Where other containers reach `smallwebwaf` directly, …; the setting, narrowed to traefik's own network, closes that gap." Fix: end the sentence at "header", then "Narrowing the setting to traefik's own network closes that gap."
- `SPEC.md` 407–410: "A request timeout that runs out while the body is still on its way answers `408` if … and `504` if it was waiting for the app." Fix: "A request timeout can run out while the body is on its way. The answer is then `408` if `smallwebwaf` was waiting for the client, and `504` if it was waiting for the app."
- `SPEC.md` 419–422: "A local database file, the free IPinfo Lite database, is the alternative …; the operator downloads and mounts it, and `smallwebwaf` never fetches it and holds no account or token for it." Fix: end the sentence at "host", then "The operator downloads and mounts it. `smallwebwaf` never fetches it and holds no account or token for it."
- `SPEC.md` 427–429: "…a setting needs lookups: a country list, a biased threshold, `SWWAF_ADD_LOOKUP_HEADERS` or an anomaly threshold per AS number." The second review asked for a list here. Fix: a bulleted list.
- `SPEC.md` 508–510: "The default gives up refusing an attack carried in a body; the rule files, the limits and the bans still apply …, and `SWWAF_WAF_BODY_LIMIT` switches body inspection on." Fix: two sentences, without the last clause, which 533 states.
- `SPEC.md` 533–536: "Form data and multipart bodies are read up to the size, the rest streaming on …; JSON and XML bodies are read only when no larger than the size, since those cannot be read in part." Fix: two sentences, the first ending "the rest streams on without being held in memory."
- `SPEC.md` 684–686: "They are what reveals a widely distributed scraper, …; a low `SWWAF_ASN_LIMIT_PERCENT` for its AS number is the response." Fix: "They reveal a widely distributed scraper, thousands of addresses at low rates each, which the per-client limits do not see. A low `SWWAF_ASN_LIMIT_PERCENT` for its AS number is the response."
- `docs/request-log.md` 69–71: "`counts`: the client's requests in the minute, hour and day …; its bytes as `minute_bytes`, `hour_bytes` and `day_bytes`." Fix: a nested list.
- `docs/gitea.md` 16–17: "…every other upload: a git push, an LFS object, a container image layer, a package file or a release attachment." Fix: a nested list.
Hard to follow on a first reading:
- `SPEC.md` 60–62: "picks the ban to drop past `SWWAF_MAX_BANS`". The first review called "dropped past" unclear. Fix: "picks the ban to drop when the table of bans is full."
- `SPEC.md` 65–66: "the six changes" appears before anything has introduced them. Fix: "with the six changes listed under "Attack detection"".
- `SPEC.md` 345–347: "…bounds the series by AS number and by country: that many of the busiest each, the rest summed as `other`, so the label set stays bounded." Fix: "`SWWAF_METRICS_TOP_N` is how many of the busiest AS numbers and countries get a series of their own. The rest are summed as `other`."
- `SPEC.md` 440–442: "`SWWAF_LOOKUP_TIMEOUT` bounds both how long a visitor's request waits for its client's first answer, when a setting needs it, and how long a request to GeoJS may take." The second review asked for a split. Fix: "`SWWAF_LOOKUP_TIMEOUT` is how long a visitor's request waits for its client's first answer. It also bounds each request to GeoJS."
- `SPEC.md` 448–449: "A service that cannot accept this, or that every new visitor's address goes to a third party, uses the file." The two halves do not fit together. Fix: "A service that cannot accept this uses the file."
- `SPEC.md` 450–451: "…sent as one `file_error` alert, and ignored, the file in use staying in use." Fix: "One that cannot be read is logged, sent as one `file_error` alert and ignored. The database already loaded stays in use."
- `SPEC.md` 544–546: "`SWWAF_ERROR_BURST_THRESHOLD` bounds the refusals per client per minute after a rule file match, …". "After" reads as a time. Fix: "…counts a client's refusals per minute for a rule file match, a trap path, a Core Rule Set match, or a missing or wrong token."
- `SPEC.md` 646–647: "DNS blocklists (`SWWAF_DNSBL_ZONES`) meant for mail list residential ranges and would block ordinary visitors". The words "mail list" read as one noun. Fix: "DNS blocklists made for mail servers list residential ranges, and would block ordinary visitors."
- `SPEC.md` 828: "Each app remains one container." "Remains" tells of a change from an earlier setup. Fix: "Each app is one container."
- `docs/reputation.md` 95–96: "at the client's first request after the offence that no ban refuses". The phrase "that no ban refuses" seems to describe the offence. Fix: "The check runs in the background at the client's next request that no ban refuses, so no request waits."
- `docs/gitea.md` 153–155: "The request is refused when the image's owner, or its name after the owner, starts with an entry on the list of commands." The first review asked for a split. Fix: "The request is refused when the image's owner starts with an entry on the list of commands. So is one whose name after the owner does."
Judgement call: some facts in `docs/` were never in the old `SPEC.md`, such as the RFC 5782 query names, the 16 MiB fetch limit and the score for more than 1000 parameters. They come from `README.md` on `next` through https://git.eeqj.de/sneak/smallwebwaf/pulls/157. I treat them as moved README content, not additions to the spec.
Judgement call, carried from the second review: `README.md` on `next` still points at `SPEC.md` sections this PR removes. https://git.eeqj.de/sneak/smallwebwaf/pulls/157 must land with this one or before it.
Not reviewed sentence by sentence: `docs/settings.md`, `docs/state-files.md`, `docs/rules.md`, `docs/bans.md`, `docs/admin-endpoints.md`, `docs/alerts.md` and `docs/metrics.md`, which another reviewer covers. I read them only for coverage and for repeats of `SPEC.md`.

FAIL: needs-rework
Line numbers are those of the new
SPEC.md.1. Not short, and many requirements are said two to four times. The new spec has about 15,100 words; the old one had about 16,500. #156 asks for a short spec that says each requirement once. Requirements said more than once:
SWWAF_prefix and why it exists (Purpose, Settings, "The two processes");SWWAF_MAX_BANSis reached ("Counting method", "Bans", theSWWAF_MAX_BANSsetting);127.0.0.1:8081(Architecture, "Admin endpoints", "Deployment", "Ports");431answer to oversized headers (twice in "Size and time limits");banaction, Risks);SWWAF_WAF_BODY_LIMIT);SWWAF_ERROR_BURST_THRESHOLD);SWWAF_RATE_LIMIT_EXEMPT_NETS(the rate limits intro, the gitea notes);SWWAF_WAF_DISABLED_RULES, "Rule files");bans.jsonwrite delay (the setting, "Writing");Acceptable: each requirement stated once where it belongs, and other places link to it. The roughly 1,500 words of "Notes specific to gitea" are observations, not requirements, as the PR body itself says. They go to
README.mdordocs/, unless the owner rules that they stay.2. Broken list at 1542. "For such a file, these requests are refused: - the API's list of its commits (
path); - …" renders as one paragraph with literal dashes. Acceptable: a real nested list.3. Non-goal and planned item contradict each other. Non-goals lists "Sharing bans between the instances of several apps.", and "Planned, not built" lists the same thing as planned. Acceptable: it appears in one place only.
4. The
offrule at 268–275 is wrong against the code. This is the first difference the PR body lists. The code also refusesoffforSWWAF_MAX_TRACKED_CLIENTS,SWWAF_MAX_BANS,SWWAF_ABUSEIPDB_DAILY_BUDGET,SWWAF_LOG_REMOTE_BUFFERandSWWAF_METRICS_TOP_N. Those are limits, yet the text says only settings that are not limits refuseoff. The code also refuses it for the percentage settings, such asSWWAF_ABUSEIPDB_MIN_SCORE. The text saysoffswitches off "a timeout", but the lookup and reputation timeouts refuse it. Acceptable: a bulleted list of exactly the settings that takeoff, a line saying every other setting refuses it, and the PR body item corrected.5. Step 7 is false. "A cached DNS blocklist verdict or AbuseIPDB score gets
SWWAF_REPUTATION_ACTION." Only a client a DNS blocklist lists, or one scored at or aboveSWWAF_ABUSEIPDB_MIN_SCORE, gets it. Acceptable: "A client listed by a DNS blocklist, or scored at leastSWWAF_ABUSEIPDB_MIN_SCOREby AbuseIPDB, getsSWWAF_REPUTATION_ACTION."6. Changes from the old spec that the PR body does not list.
SWWAF_IPV6_GROUP_PREFIXrange of 32 to 128;offswitching off the alert cooldown; the trap path in ban notes; refusals as a kind of limit hit, and dropped alerts, in the metrics; thesummaryevent in the webhook schema.bans.jsonandlookups.json; the list of requestsscript/gitea-requestssends; that the app can read a token given as an environment variable, which is the reason for the file form.#156 asks for every difference to be listed rather than settled silently. Acceptable: each one listed in the PR body, or restored.
7. Dangling references. The build order is gone and "Configuration surface" has been renamed, but other files still point to them:
README.mdpoints to "the build order inSPEC.md" at about lines 69 and 1983, and in its TODO.internal/waf/waf.goandinternal/anomaly/anomaly.gopoint to sections "under "Configuration surface"".Acceptable: these references updated in this PR.
8. PR body length. It has about 390 words; the limit is about 250. Acceptable: a one-sentence intro and one short line per difference.
9. Failing sentences.

Line numbers are those of the new
SPEC.md. Every finding of the first review is fixed, apart from theREADME.mdpart of finding 7, which is fairly answered by #157 (see the disclosure at the end).1. Requirements still said more than once. #156 asks for each requirement once. These still appear in two to five places:
bans.jsonwrite delay: the setting (291) and "Writing" (927);SWWAF_UNKNOWN_LIMIT_PERCENT(491);blockaction (776), Risks (1360);banpath rules anchored at the site root: 779, 828, Risks (1353);/_smallwebwaf/never reaching the app: 48, step 13 (110), 1074;bans.json: 201, 275, 953, 1086, 1102, 1327;Acceptable: each stated once where it belongs, with a link from anywhere else that needs it.
2. Sections that add no requirement of their own.
SWWAF_BAN_RESPONSEandSWWAF_BAN_SCOPE_V4_PREFIX. Give the four length settings as names that link to "Bans".README.md.docs/gitea.md.3. The
offrule is false (233). "Every other setting refusesoff."SWWAF_LOOKUP_SOURCEandSWWAF_WAF_MODEboth takeoff. Acceptable: add both to the list.4.
Acceptable: "It ends when an admin lifts it, or when
smallwebwafdrops it from a full table, as below."6. Misleading (681). "its decision list is pulled on a schedule and treated as a blocklist". That reads as if
SWWAF_BLOCKLIST_ACTIONapplies. Acceptable: "It is kept like a fetched blocklist, and a client on it is banned (see "Bans")."7. Requirements dropped without a note in the PR body. Acceptable for each: restore it in one short sentence, or list it in the PR body.
SWWAF_WAF_ANOMALY_THRESHOLD. Also missing: withoff, nothing is refused.smallwebwafpasses bodies through unchanged" does not say this.Refererchange is incomplete.8. SPEC.mdkeeps the requirement and its reason. A docs file keeps only the detail it adds, and links to theSPEC.mdsection for the rest.2. The settings groups do not match (
SPEC.md291–292). The text says "docs/settings.mdlists every setting … in the groups below.", but that file's groups have other names and other contents. One concept also has three names: "Biased thresholds" inSPEC.md, "Lower limits by AS number and country" indocs/settings.md, and "the lower limits" indocs/lookup.md66 and 109. Acceptable: the same group names in both files, and one name for each concept.3. False or misleading statements.
SPEC.md278: "Every other setting refusesoff." Free-text settings takeoffas an ordinary value, for exampleSWWAF_INSTANCE_NAME,SWWAF_LOG_REQUEST_HEADERSandSWWAF_RULES_DIR. Acceptable: "No other setting can be switched off withoff."SPEC.md313–315: "SWWAF_METRICS_TOKENswitches on the metrics, which would otherwise be open to anyone on the internet." This reads as if the metrics are open without the token, but without it they answer404. Acceptable: "The metrics are off untilSWWAF_METRICS_TOKENis set, since without a token they would be open to anyone on the internet."SPEC.md351: "SWWAF_ALLOW_NETSbypasses everything". Step 3 says such a client is still counted for the anomaly thresholds and still needs a token. The size and time limits still apply to it too. Acceptable: "SWWAF_ALLOW_NETSskips the checks step 3 names, for monitoring or the owner's own networks."SPEC.md179–181 says the fixed bounds are under "Fixed limits" indocs/settings.md. The anomaly counters' bound is not there;SPEC.md693 anddocs/settings.md443 both give it. Acceptable: the bound in one place, and a pointer that is correct.SPEC.md751: "Each file is rewritten whole, so each has a bound given by its setting." Cause and effect are reversed. Also, fixed limits, not settings, boundlookups.json,reputation.jsonandalerts.json. In 752, "these sizes" points at sizes now given only indocs/state-files.md. Acceptable: "Each file has a bound, so it can be rewritten whole."docs/reputation.md6–8: "…a source the client is on refuses it, lowers its limits, or only notes it, asSWWAF_BLOCKLIST_ACTIONandSWWAF_REPUTATION_ACTIONsay." The CrowdSec decision list bans instead, and the sentence runs past 30 words. Acceptable: "A blocklist acts asSWWAF_BLOCKLIST_ACTIONsays. A DNS blocklist or AbuseIPDB acts asSWWAF_REPUTATION_ACTIONsays. The CrowdSec decision list bans."4. Requirements said twice within

internal/state/state_test.go1023 cites "Edits while running" inSPEC.md, which no longer exists.internal/state/state.go2 says the "Persistent state" section describes what each file holds; that list is now indocs/state-files.md. Acceptable: both point atdocs/state-files.md.7. Failing sentences.
Over about 30 words, or a list chained with commas:
SPEC.md7–9, in the opening: "The reference documents underdocs/, which this document links to, give the settings, the formats and the lists; every fact lives in one of these files and is linked from the others." The verb also comes late. Fix: "The reference documents underdocs/give the settings, the formats and the lists. This document links to them."SPEC.md305–307: "Where other containers reachsmallwebwafdirectly, …; the setting, narrowed to traefik's own network, closes that gap." Fix: end the sentence at "header", then "Narrowing the setting to traefik's own network closes that gap."SPEC.md407–410: "A request timeout that runs out while the body is still on its way answers408if … and504if it was waiting for the app." Fix: "A request timeout can run out while the body is on its way. The answer is then408ifsmallwebwafwas waiting for the client, and504if it was waiting for the app."SPEC.md419–422: "A local database file, the free IPinfo Lite database, is the alternative …; the operator downloads and mounts it, andsmallwebwafnever fetches it and holds no account or token for it." Fix: end the sentence at "host", then "The operator downloads and mounts it.smallwebwafnever fetches it and holds no account or token for it."SPEC.md427–429: "…a setting needs lookups: a country list, a biased threshold,SWWAF_ADD_LOOKUP_HEADERSor an anomaly threshold per AS number." The second review asked for a list here. Fix: a bulleted list.SPEC.md508–510: "The default gives up refusing an attack carried in a body; the rule files, the limits and the bans still apply …, andSWWAF_WAF_BODY_LIMITswitches body inspection on." Fix: two sentences, without the last clause, which 533 states.SPEC.md533–536: "Form data and multipart bodies are read up to the size, the rest streaming on …; JSON and XML bodies are read only when no larger than the size, since those cannot be read in part." Fix: two sentences, the first ending "the rest streams on without being held in memory."SPEC.md684–686: "They are what reveals a widely distributed scraper, …; a lowSWWAF_ASN_LIMIT_PERCENTfor its AS number is the response." Fix: "They reveal a widely distributed scraper, thousands of addresses at low rates each, which the per-client limits do not see. A lowSWWAF_ASN_LIMIT_PERCENTfor its AS number is the response."docs/request-log.md69–71: "counts: the client's requests in the minute, hour and day …; its bytes asminute_bytes,hour_bytesandday_bytes." Fix: a nested list.docs/gitea.md16–17: "…every other upload: a git push, an LFS object, a container image layer, a package file or a release attachment." Fix: a nested list.Hard to follow on a first reading:
SPEC.md60–62: "picks the ban to drop pastSWWAF_MAX_BANS". The first review called "dropped past" unclear. Fix: "picks the ban to drop when the table of bans is full."SPEC.md65–66: "the six changes" appears before anything has introduced them. Fix: "with the six changes listed under "Attack detection"".SPEC.md345–347: "…bounds the series by AS number and by country: that many of the busiest each, the rest summed asother, so the label set stays bounded." Fix: "SWWAF_METRICS_TOP_Nis how many of the busiest AS numbers and countries get a series of their own. The rest are summed asother."SPEC.md440–442: "SWWAF_LOOKUP_TIMEOUTbounds both how long a visitor's request waits for its client's first answer, when a setting needs it, and how long a request to GeoJS may take." The second review asked for a split. Fix: "SWWAF_LOOKUP_TIMEOUTis how long a visitor's request waits for its client's first answer. It also bounds each request to GeoJS."SPEC.md448–449: "A service that cannot accept this, or that every new visitor's address goes to a third party, uses the file." The two halves do not fit together. Fix: "A service that cannot accept this uses the file."SPEC.md450–451: "…sent as onefile_erroralert, and ignored, the file in use staying in use." Fix: "One that cannot be read is logged, sent as onefile_erroralert and ignored. The database already loaded stays in use."SPEC.md544–546: "SWWAF_ERROR_BURST_THRESHOLDbounds the refusals per client per minute after a rule file match, …". "After" reads as a time. Fix: "…counts a client's refusals per minute for a rule file match, a trap path, a Core Rule Set match, or a missing or wrong token."SPEC.md646–647: "DNS blocklists (SWWAF_DNSBL_ZONES) meant for mail list residential ranges and would block ordinary visitors". The words "mail list" read as one noun. Fix: "DNS blocklists made for mail servers list residential ranges, and would block ordinary visitors."SPEC.md828: "Each app remains one container." "Remains" tells of a change from an earlier setup. Fix: "Each app is one container."docs/reputation.md95–96: "at the client's first request after the offence that no ban refuses". The phrase "that no ban refuses" seems to describe the offence. Fix: "The check runs in the background at the client's next request that no ban refuses, so no request waits."docs/gitea.md153–155: "The request is refused when the image's owner, or its name after the owner, starts with an entry on the list of commands." The first review asked for a split. Fix: "The request is refused when the image's owner starts with an entry on the list of commands. So is one whose name after the owner does."Judgement call: some facts in
docs/were never in the oldSPEC.md, such as the RFC 5782 query names, the 16 MiB fetch limit and the score for more than 1000 parameters. They come fromREADME.mdonnextthrough #157. I treat them as moved README content, not additions to the spec.Judgement call, carried from the second review:
