TODO.md rewritten sentence by sentence

TODO.md is rewritten from scratch so that every sentence reads on the first pass: one idea each, under about 30 words, the verb early. Workflow, Next Step and Future Steps keep their substance. Each Completed Steps entry keeps its date, its issue link and what changed; entries describing a state since changed are in the past tense. Part of #189.
