Replaces the old flag-file poller documentation with the current dispatcher pattern:

Poller is now the dispatcher itself
Spawns one isolated agent per actionable notification via openclaw cron add --session isolated
Includes assignment scan backup loop
Adds scope enforcement, priority rule, and security patterns
Documents lessons learned (cron format, session routing, scope enforcement)
All credentials and config values sanitized to placeholders
