Carries out the decision on #222: a change no longer adds an entry to Completed Steps in TODO.md.
Each change used to add an entry at the top of Completed Steps in its own commit, so every merge into next left every other open PR conflicting on that one list. The Workflow section now says to add no entry. What landed is recorded on the tracker and in git log, where each commit title names the issue it closes.
The step "Move Next Step to the top of Completed Steps; move the top item of Future Steps into Next Step" is removed whole. Its first half added the same entry in another way, and its second half only refilled the slot the first half emptied. Next Step itself is untouched.
The entries already in Completed Steps stay, as the plan on the issue says. The Workflow does not say who writes to Completed Steps from now on, or when; the plan did not ask for that.
Judgement call: the clause "with the TODO.md update in that same commit" is dropped, because that update was the entry.
