Every fix from the defect hunt in #288 that added its own Completed Steps entry to TODO.md put it at the same spot, so each merge to next made every other open PR conflict there. From now on the fix PRs leave TODO.md alone, and their entries are written together here. PRs already open keep the entries they have.
Definition of done
After the last fix from the hunt lands, TODO.md has a Completed Steps entry, in the style of the existing ones, for every issue from the hunt that was closed by a PR and has none yet, newest first.
The "Next Step" section is true for the tree at that point.
Docs only; one PR on next, reviewed like any other.
